<p><b>Physician Assistant OR Nurse Practitioner Green Bay WI</b></p> <p><span style="background:white">We are looking for an experienced Physician Assistant or Nurse Practitioner to join our group in Green Bay, WI. We are looking for a dynamic, clinically adept Nurse Practitioner or Physician Assistant to work with our Physician leaders at a rehab hospital. Additionally, you would collaborate with corporate leadership to assist in maintaining optimal momentum of our stellar quality metrics, program components and implementing our proprietary protocols to achieve clinical outcomes. <span style="background:white">We are seeking a dynamic and talented NP/PA to join our team! Must have <b>a minimum 3 to 5 years of acute inpatient hospital experience as a physician assistant or a nurse practitioner and the ability to work autonomously</b></span></span></p> <p><span style="background:white"><b>Responsibilities:</b></span></p> <ul> <li><span style="background:white"><span style="tab-stops:list .5in">History and Physical Exams- Examine patients and their medical records, pertinent documents/imaging/labs/test</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Order and study diagnostic tests (blood tests, X-rays etc.)</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Diagnosis, management and treatment of acute episodic illnesses and stable chronic conditions, evaluation of current therapies, health maintenance, health promotion, illness prevention education and counseling, and management of urgent care matters</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Prescribe medications</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Write referrals and consult other physicians for comprehensive patient care</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Maintain accurate records and schedules- Establishes and maintains a system for tracking and coordinating the designated patient caseload and their progress throughout the entire acute or chronic episode of care</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Inform patients about their health conditions and prescribed medications</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Educate patients and their family members on diseases, epidemics and prevention- teaching and counseling to patients and families</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Collaboration with the healthcare team- Monitors, evaluates, and communicates relevant data regarding clinical problems to Physician, as appropriate and members of the interdisciplinary team</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Oversee patients’ overall health- identify opportunities for each patient's needs relative to his/her age, Monitors progress of patients and collaborates with other team members to ensure continuity of care.</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Promote good health practices</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Identifies and addresses potential deterrents to compliance with treatment regime.</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Advance professional knowledge and conduct research</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Follows up with patients if they are discharged to assess if needs are being met and determines if additional follow-up care is required.</span></span></li> </ul> <p><span style="background:white"><b>Skills:</b></span></p> <ul> <li><span style="background:white"><span style="tab-stops:list .5in"><b><span style="background:white">Minimum 3 to 5 years of acute inpatient hospital experience as a physician assistant or a nurse practitioner is required and ability to work autonomously.</span></b></span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Expertise in primary care, acute care and chronic care practices</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Valid nurse practitioner or physician assistant license in WI</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Master’s degree (graduation from an accredited NP or PA program)</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">ACLS Certified</span></span></li> </ul> <p><b>Schedule:</b></p> <ul> <li>Full time</li> </ul> <p style="margin-left:24px"> </p> <p><b>Compensation:</b></p> <ul> <li><b>$120k</b></li> </ul> <p> </p> <p><b>Benefits:</b></p> <p> </p> <ul> <li>Health, Dental, & Vision Benefits</li> <li>Retirement program with matching</li> <li>Paid Time Off</li> </ul> <p><span style="background:white">We are seeking a caring provider with excellent communication skills, someone with team spirit and a positive attitude, to join our well-respected team.
