HCBS Coordinator. Care with Heart, Presence, and Purpose. As the Home and community-based coordinator, you are responsible for Home and community-based services and In-home support services intake process to ensure that all state and federal regulations are met, and company policies are upheld. Responsible for establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns. Assisting with Human Resource functions.
What You'll Do
  • Coordination with case managers to ensure the delivery of safe, effective and appropriate home care services.
  • Completes intakes for PCP/Homemaking/IHSS operations including providing direct oversight of the establishment and implementation of intake policies.
  • Ensure compliance with all state and federal referral/intake regulatory requirements
  • Establishes and maintains positive working relationships with current and potential referral sources.
  • Provides supervisory, performance management counseling and annual evaluations of care providers
  • Performs non-skilled competencies and required training to new and existing employees

What We're Looking For

Pay Rate: $23.00 - $26.00/hourly
