Job description

RISE is an innovative human services network originally established in 1987 for the purpose of moving individuals living in institutions into family settings. Since that time, services have grown to support children, adults, and families with a variety of needs across multiple states that include residential settings, day programs, employment assistance, managed care, and home and community based services. Since inception, our purpose and mission has remained constant. We create opportunities for and with the people we serve, their families, and for the men and women who work in the direct care profession.
RISE has an exciting opening to hire an IDLA Home Team Lead Direct Support Professional. The position is considered a safety sensitive position and will be responsible for the management of assigned residential supports through the implementation of the RISE mission, vision and values and in compliance with all regulations and policies. This is an excellent opportunity for a dedicated professional experienced in working with individuals with intellectual and/or developmental disabilities and the elderly to provide leadership and support to our Direct Support Professional staff along with the people and families we serve.
Some of the responsibilities with this position are:
  • Provide team lead support in a manner that meets and exceeds the expectation of RISE customers
  • Responsible to coach, train, develop and discipline direct support professionals
  • Assures that support provided by Direct Support Professionals meet expectations outlined in all RISE and State Policies and Procedures
  • Comply with all DES/DDD and other relevant **Licensing Requirements
  • Assure that RISE is a learning organization at all levels and is continually looking for ways to increase knowledge and skills
  • Maintain necessary records, logs, and other documentation as required by the planning teams, RISE, and the state of Arizona DES DDD division
  • Attend all staff meetings and other mandatory training as required
  • Schedule and conduct regular staff meetings
  • Work with HCBS coordinator to schedule matrix to meet staffing ratios
  • Assure all staff are in compliance with DCI-Direct Care Intervention time and attendance procedures.
  • Demonstrate competency in financial and budgetary issues
  • Demonstrate respect and promote choice for those whom we support; daily activities shall be based on the person's choice and plan's outcomes
  • Promote the rights of people receiving support and contribute to an environment that is free of abuse, neglect and/or exploitation
  • Establish partnerships with RISE customers, i.e. people receiving support, supervisor, staff, each respective family/guardian and funding agencies
  • In conjunction with the each person's planning team, assist the person in identifying and achieving his or her plan outcomes; plans shall be reviewed with the team on a quarterly basis
  • Support people in achieving outcomes identified in the person's plan
  • Maintain all required documentation and any other prescribed or related task
  • Evaluate the effectiveness of each person's plan on a weekly basis; if the plan is not meeting the person's needs, will coordinate a team meeting to reevaluate the plan and develop a solution
  • Complete monthly summaries for each person and forward to the families and Support Coordinator by the 10th of each month
  • Ensure that people are assisted (as relevant to the person) with supports of daily living, accessing the community, medication and medical issues when indicated
  • Act as an appropriate role model and support people in using appropriate language, dress, manners, and respect
  • Ensure that clean and safe environments are maintained
  • Quickly and effectively, respond to emergencies such as fire, application of First Aid, CPR, approved medical protocols, behavior intervention and etc.
  • Frequently interacts with people receiving support

  • Minimum of High School Diploma or GED and a combination of two (2) years supervising staff and experience working in a related field; an Associate Degree and a combination of two (2) years of supervising staff and experience working in a related field is preferred
  • Must have verbal and written communication skills adequate to the specific job
  • Must be at least 21 years of age and meet requirements of RISE vehicle insurance agency if required to drive a RISE Vehicle
  • Satisfactory background clearance and obtain Level One Fingerprint Card
  • Language ability (other than English) may be required to meet specific needs of the program(s)
  • Self-directed, works with minimal supervision
  • Ability to work flexible hours
  • First Aid/CPR certified

Additional Candidacy Requirements:
  • Must have a valid Arizona driver's license, minimum of twenty-one years of age and meet driver requirements as specified by RISE Insurance Agency
  • Must be able to complete a successful background check, driving record check, drug test and obtain a Level One Fingerprint Clearance Card
