Are you ready to make a real difference in people's lives? Join the Harris Center for Mental Health and IDD as a Care Coordinator. Under the general direction of the Clinical Team Lead, Care Coordinator (CC) provides case management and coordination of care services. The Care Coordinator will provide all services within the community for individuals participating in a housing program affiliated with the Coalition for the Homeless. The CC will provide care coordination and case management services as part of a multidisciplinary team that includes a Master Level Clinician or LPHA and LCDC. Peer support services are also offered within the program. The CC will be expected to provide transportation to consumers often. Through the provision of intensive crisis intervention, engagement, and care coordination services, maintaining housing and mental health is the primary focus for this program. The Care Coordinator must hold a valid, State of Texas driver's license and required insurance coverage. Staff are required to transport consumers using their personal vehicle and will be reimbursed for mileage by the Agency.
Your Role in Action
As a Care Coordinator for this unit, you will be:
- • Performing clinical and financial assessments to determine service eligibility and consumer needs.
- • Ensuring all services provided meet eligibility criteria and are authorized in accordance with agency and regulatory guidelines.
- • Providing interventions in response to crisis to reduce symptoms and avoid admission to a more restrictive environment and ensures appropriate service referrals are completed.
- • Performing activities to assist consumers in gaining and coordinating access to necessary care and services based on individual needs.
- • Providing education on Mental Health and related resources.
- • Monitoring consumer progress with treatment goals and linkage to services.
- • Providing and assist with consumer activities of daily living.
- • Meeting divisional productivity standards as outlined in the mental health division's operations guidelines.
- • Completing/Authorizing consumer's service plan/level of care assignment to address identified needs and ensures linkage to related services.
- • Completing reporting in an accurate and timely manner as designated.
- • Advocating for consumers to ensure that needs are met both within the Harris Center system and in the community.
- • Communicating and works with the patients, visitors, and other staff by following the key principles of a Trauma Informed approach (i.e. safety, trust worthiness, transparency, peer support, empowerment, voice & choice, culture, history & trauma).
- • Performing related work as assigned and adapts to changing work requirements.
- • Demonstrating ethical conduct in all clinical and professional activities.
What qualifications you will need: Education : - Bachelor's degree (B.A.) from four-year accredited college or university with a major in a social, behavioral, or human service field, including but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, and educational psychology, required.
Experience: - Minimum of one year experience in the mental health field, preferred.
Other Key Qualifications - Must maintain a valid Texas Driver's License and be insurable under Agency policy.
- The ideal candidate will be required to transport consumers using their personal vehicle and will be reimbursed for mileage by the Agency.
Knowledge, Skills & Abilities: - Basic understanding of mental illness, common signs and symptoms and able to identify difficulties with coping in community settings.
- Strong communication skills, listening and interpersonal skills above average, written documentation is clear, concise, well- constructed and grammatically correct.
- Able to work collaboratively with others in a teamwork environment.
- Effective time management and organizational skills. Able to execute standard word processing.
- Must be able to work autonomously in a fast-paced environment.
