Position Summary

We are seeking a highly motivated, Personal Health Nurse to join our team. This full-time, hybrid role combines in-person visits at our client sites with remote care coordination and telehealth support.

Key Responsibilities

  • Provide direct nursing care, health assessments, and chronic disease management to clients.

  • Conduct in-home or onsite visits and deliver telehealth telephonic support as needed.

  • Develop individualized care plans, coordinate services, and monitor progress.

  • Educate clients and families on preventive health, medication adherence, and self-management.

  • Collaborate with interdisciplinary teams, community partners, and other healthcare providers.

  • Maintain accurate electronic health records and comply with HIPAA and other regulatory standards.

Qualifications

  • Current RN license (State of DE) in good standing.

  • Bachelor’s degree in Nursing preferred.

  • Bilingual fluency in English and Spanish is a plus!

  • Minimum 2 years of nursing experience, preferably in community health, primary care, or care coordination.

  • Strong communication, cultural competence, and organizational skills.

  • Proficiency with electronic health records and telehealth platforms.


Schedule & Work Environment

  • Full-time (40 hours per week), Monday–Friday.
