Job description

Social Services Assistant

We are seeking a compassionate and dedicated Social Services Assistant to join our Skilled Nursing Facility team. In this role, you will support the Director of Social Services in delivering high‑quality, resident‑centered care that enhances each resident’s quality of life and overall well‑being.

The Social Services Assistant plays a key role in planning, developing, organizing, implementing, and evaluating social services programs that support residents and their families throughout the length of stay.

Key Responsibilities
  • Assist in planning and implementing a comprehensive Social Services program that provides counseling, support, and advocacy for residents and their legal representatives.

  • Identify medical‑related social needs of residents and provide appropriate services to meet both individual and collective needs.

  • Maintain accurate and timely documentation related to residents’ social service needs, care plans, and interventions.

  • Collaborate with the Director of Social Services and interdisciplinary team members regarding resident concerns and care coordination.

  • Work directly with residents and their legal representatives who are experiencing personal, emotional, or environmental challenges related to physical or emotional conditions.

  • Support compliance with federal, state, and facility regulatory requirements related to social services.

Qualifications
  • Bachelor’s Degree in Social Work preferred; Bachelor’s Degree in Human Services or related field will be considered.

  • Previous experience in social services required.

  • Prior long‑term care or skilled nursing experience strongly preferred.

  • Must possess any certifications or licensure required by state regulations.
