Job description

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions, development/intellectual disabilities and homelessness to help stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.
We are seeking a qualified candidate who is interested in joining our Case Management Assertive Community Treatment (ACT), in Georgetown, DE.
The Mental Health Specialist as part of the ACT multidisciplinary team provide rehabilitation and support functions and assist in treatment, provide substance abuse services, education, support and consultation to families; and provide crisis intervention under clinical supervision.
Some of the responsibilities would include the following:
  • Provide clinical assessment and consultation to staff and clients regarding client goals and follow-up.
  • Participate in the analysis and identification of service delivery gaps; changes and problems under the guidance of the Team Leader and implement appropriate action plans with program staff.
  • Act as a liaison and consult with community agencies and families.
  • Coordinate the provision of emergency services and hospital liaison services when a consumer is in crisis and work with community agencies to develop needed resources including housing, employment options and income assistance.
  • Maintain a permanent record of participants activates according to established procedures.
  • Provide counseling to help the participants cope with and gain mastery over symptoms and disabilities in the context of daily living.

Qualifications:
  • A master's degree in Nursing, Social Work, Counseling, Psychology, Healthcare or related human service field.
  • At least 2 years of experience in the MH/MR field; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.
  • Substance abuse experience helpful.
  • Excellent communication skills both oral and written.
  • Detailed oriented and very strong organization skills.
  • Strong counseling and direct service skills.
  • Ability to work well as a member of a team and with all levels of staff.
  • Possess a valid driver's license with an acceptable driving record.
  • Ability to travel to several residential locations.

Scope:
  • Frequent talking and listening.
  • Requires the ability to lift, carry, fold, kneel, reach, stoop and bend which may include direct intervention with clients.
  • Possess manual dexterity and fine motor skills.
  • Must be available for local travel.
  • May require crisis intervention in emergency situations.
  • May be assigned to complete 24 hours on call beeper coverage and assigned/needed.
  • Must be available to work flexible hours, in accordance with client needs and scheduling.
