Job description

JOB PURPOSE: 

The Case Manager/Outreach & Intake Specialist, under the direction of the Program Manager, provides comprehensive outreach, intake, case management, employment, and supportive services to Veterans participating in the Homeless Veterans' Reintegration Program (HVRP). The position is responsible for conducting community outreach, determining program eligibility, completing participant enrollment, developing individualized employment and service plans, coordinating supportive services, and assisting participants in obtaining and maintaining meaningful employment. 

ESSENTIAL JOB RESPONSIBILITIES 

  • Conduct community outreach and represent HVRP at community meetings, resource fairs, shelters, partner agencies, and Veteran-serving organizations to recruit eligible participants and increase community awareness.  

  • Conduct eligibility screenings, determine HVRP eligibility, complete participant intake and enrollment, and establish participant files in accordance with program requirements.  

  • Complete comprehensive assessments to identify employment barriers, housing needs, supportive service needs, education, training, and career goals.  

  • Provide ongoing case management throughout program participation, monitoring participant progress toward employment, housing stability, and self-sufficiency.  

  • Assist participants in accessing employment opportunities, vocational training, apprenticeships, educational programs, certifications, and other workforce development resources.  

  • Provide employment services including resume development, interview preparation, job search assistance, employer referrals, application assistance, and job retention coaching.  

  • Develop and maintain relationships with employers, workforce agencies, educational institutions, landlords, property management companies, and community partners to increase employment and housing opportunities.  

  • Connect participants with housing resources, healthcare, behavioral health services, VA benefits, transportation, financial assistance, and other community resources.  

  • Assist participants with obtaining identification documents, benefits, supportive services, budgeting, financial literacy, and life skills necessary for long-term stability.  

  • Maintain accurate and timely documentation including assessments, service plans, case notes, employment documentation, incident reports, and required program documentation within Salesforce, and other required databases.  

  • Prepare reports and maintain documentation necessary for grant compliance and performance outcomes.  

  • Demonstrate exceptional customer service by placing Veterans and their families first while supporting Endeavors' mission to empower people to build better lives for themselves, their families, and their communities.  

  • Perform other duties as assigned.  

Essential Qualifications 

Education: Bachelor's degree in Social Work, Psychology, Public Administration, Human Services, or a related field preferred; equivalent combination of education and experience may be considered.  

Experience 

  • Minimum one (1) year of experience in case management, workforce development, employment services, Veteran services, homelessness programs, or other human service programs.  

  • Experience working with Veterans, individuals experiencing homelessness, employment placement, or behavioral health populations preferred.
