Housing Case Workers focus on assisting clients with housing needs, often working in community or shelter settings. POSITION SUMMARY POSITION SUMMARY: The primary goal of the Housing Case Worker II is to provide permanent and affordable housing resources and linkages to appropriate services to individuals who are experiencing homelessness or at risk of homelessness and who may have complex health conditions. The Housing Case Worker II will assist, and support homeless individuals enrolled in Yurok Tribe's housing programs. The Housing Case Worker II works directly with program participants and develops, coordinates treatment services, assists participants in identifying and securing housing, provides housing retention support, and other support services.
DUTIES & RESPONSIBILITIES DUTIES AND RESPONSIBILITIES: 1. Conducts initial intake with potential program participants to determine program eligibility. 2.
Conducts comprehensive assessments and uses intake forms to determine appropriate housing options, case management and other supportive service needs. 3. Conducts HMIS/CES assessment and enters participants into HMIS/CES 4.
Develop individualized case plans with measurable goals and objectives to assist participants in achieving their desired outcomes. 5. Conducts on-going quarterly needs and financial assessments to identify client's continued housing plan, supportive service needs and barriers to securing/maintaining permanent supportive housing.
6. Works with program participants to develop a housing stability plan and budget (this includes a service plan). 7.
Works with program participants and the assigned Housing Navigator to develop a housing location plan. 8. Assists participants to get "document ready" by supporting them to obtain IDs, Social Security cards, birth certificates, and other key documents needed to successfully obtain housing and link to appropriate supportive services, including letters of reference for landlords.
9. Assists participants with securing and/or increasing income (once housed) including connections to mainstream benefits. 10.
Networks and coordinates with representatives from other agencies & community-based organizations to support individuals in attaining services such as housing, mental health care, medical treatment, financial assistance, legal advocacy, etc. 11. Identifies long-term affordable housing resources (such as local housing authority applications for Housing Choice Vouchers, connections to Tribally managed housing units, etc.) and assist clients to pursue options with referrals and support filling out applications
12. Coordinates housing search activities with the Housing Locator and attends unit/apartment viewings with participants. 13.
Educates participants on tenant rights, responsibilities, and how to be a good tenant. 14. Provides tenant support when housing-related issues occur.
15. Assists participants with transitioning into permanent housing and provides supportive services to improve participants overall housing stability. 16.
Performs home visits, eviction prevention, budgeting, life skills coaching, and other housing retention services to support clients with sustaining permanent supportive housing. 17. Prepares, maintains, and updates clients' hard case files/e-files related to the case for review and record keeping purposes.
Ensures timely completion of client case notes. 18. Models Yurok Tribe's approach, mission, and core values (including Housing First and Harm Reduction) in all communication, correspondence, community events, coalitions and advocacy efforts.
19. Reports to work on time and maintains reliable and regular attendance. 20.
Performs other related duties as assigned. EDUCATION/EXPERIENCE/MINIMUM QUALIFICATIONS EDUCATION/EXPERIENCE: 1. G8: Bachelor's degree in related field with one year job specified experience.
2. G9: Bachelor's degree in related field with two years' job specified experience. 3.
Employees must be willing to acquire any special certifications or attend any training that is determined to be relevant or become required of the position in the future. 4. Candidates with equivalent combinations of education, training and experience will be considered.
MINIMUM QUALIFICATIONS: 1. Two years of experience in human services, social work, medical, or related field. 2.
Ability to gather complete information from clients in a telephone interview or in person interview. 3. Ability to respond to clients in a calm, empathetic and professional manner while courteously directing the conversation to pertinent issues.
4. Excellent written and oral communication skills. 5.
Ability to work independently as well as support team efforts, and to strategize and plan to timely and consistently support clients to find and maintain housing. 6. Excellent organizational skills to handle an individual caseload of housing case management clients.
7. Microsoft Office skills. 8.
Familiarity with the principles of harm reduction and housing first. 9. Have a good understanding of case management and development of case plans for clients.
10. Knowledge of the laws, rules and regulations including policies, procedures, programs, and goals of public social service agencies. 11.
Knowledge and skills in the principles of supervision, training and instructional methods and techniques. 12. Ability to exercise sound judgment when organizing, directing, prioritizing, and assigning activities including selecting, training, supervising, evaluating, and disciplining subordinate staff.
13. Demonstrated strong computer skills with three plus years using Microsoft Office or similar software and including a strong working knowledge of automated database systems such as MS Access or similar database. 14.
Recognition that in order to serve Yurok People one must know as much as possible about Yurok history, culture and values. SUPERVISORY RESPONSIBILITIES/CONDITIONS OF EMPLOYMENT SUPERVISORY RESPONSIBILITIES: This position does not have supervisory responsibilities. CONDITIONS OF EMPLOYMENT: 1.
