• Collaborate closely with healthcare professionals to develop comprehensive treatment plans, inclusive of psychosocial assessments and evidence-based social work interventions.
• Conduct thorough assessments and interventions for patients to identify their needs and create actionable treatment strategies, ensuring all records, including progress notes, are accurately maintained in accordance with department policies.
• Provide individual and group counseling sessions, focusing on crisis intervention and mental health support for patients dealing with emotional and behavioral challenges.
• Engage and coordinate with families or support systems to enhance understanding and involvement in the client’s treatment and discharge planning process.
• Monitor and evaluate patient progress, making necessary adjustments to treatment plans based on their evolving needs and feedback.
• Participate in staff meetings and development programs to continually enhance skills and knowledge in the field of clinical social work.

As a vital member of our community health clinic, you will work with a diverse patient population, including individuals facing mental health disorders, emotional challenges, and social issues affecting their overall well-being. • A Master's degree in Social Work from an accredited institution, with a focus on clinical practice and counseling.
