  • Will see patients in a variety of community settings.
  • Admit patients, and establish care goals.
  • Establishes and maintains the plan of care and reviews progress to goals.
  • Coordinated Care with providers and interdisciplinary teams
  • Assess, treat, implement, and evaluate patients.
  • Seeking Nurses for long-term care.
  • Administers medications according to provider orders.
  • Delegate Care to Caregivers in community settings.
  • Comfortable providing education to nurses, caregivers, families, and patients.

Qualifications/Educational Requirements:
  • Current Washington state RN/LPN license in good standing
  • Current BLS
