At Transitions Hospice, we've reimagined what hospice looks like - compassionate, unhurried, and built around the whole person. As our Post Acute Care Liaison, you'll be the bridge between our hospice team and the hospitals and post-acute settings that trust us with their patients. What You'll Do

You will be responsible for establishing and maintaining relationships with hospitals and post-acute care settings - including nursing homes, rehabilitation centers, and assisted living facilities - to promote our hospice services and drive patient referrals. You will serve as a key point of contact, increasing awareness, educating providers on our services, and ensuring a steady, dignified flow of patient admissions.

  • Build and maintain strong relationships with hospital administrators, physicians, nurses, and other healthcare professionals to promote our hospice services
  • Develop and execute a strategic sales plan to generate and grow referrals from hospitals and post-acute care settings
  • Conduct regular visits to facilities to meet with staff, share educational materials, and facilitate smooth patient referrals
  • Collaborate with the hospice team to ensure seamless admissions for patients referred from hospitals and post-acute care settings
  • Organize and participate in healthcare events, conferences, and seminars to increase awareness within the medical community
  • Monitor and analyze referral trends, competitor activities, and market conditions to identify growth opportunities
  • Deliver timely, accurate reports on sales activities, outcomes, and challenges to the management team
  • Stay current on industry regulations, healthcare policies, and hospice-related developments
  • Partner with the marketing team to develop materials and campaigns targeting post-acute settings
  • Participate in evening and weekend call for admissions as required, in a competent and responsive manner

Requirements

What You Bring

  • Bachelor's degree in business, healthcare administration, marketing, or a related field - or equivalent experience (preferred)
  • Proven track record of success in sales, preferably in the healthcare or hospice industry
  • Knowledge of the hospice industry, end-of-life care, and related medical terminology
  • Exceptional communication and interpersonal skills to build and maintain professional relationships with healthcare providers
  • Strong negotiation and presentation skills to effectively promote hospice services and handle objections
  • Ability to work independently and as part of a team, with excellent time management and organizational abilities
  • Willingness to travel frequently to visit hospitals and post-acute care settings within the assigned territory
  • Demonstrated compassion and empathy for patients and their families facing end-of-life challenges
  • Proficiency in CRM and sales software to track leads, manage accounts, and analyze data
  • Ability to work comfortably in an interdisciplinary care setting
