Qualifications:
  • Certified Nursing Assistant (CNA) license (required)
  • 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
  • Current CPR certification (preferred)
  • Excellent customer service skills
  • Strong communication (written, verbal, and active listening)
  • Problem-solving skills
  • Time management skills
  • Ability to work in an electronic medical record system
  • Ability to lift 50 lbs.
  • Good understanding of body mechanics
  • Strong understanding of professional boundaries
  • Valid driver's license (required)

Responsibilities:
  • Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.)
  • Review patient care plans
  • Assist with lifting and transferring patients
  • Assist with range of motion, positioning, exercise, and ambulation
  • Assist with dining and nutrition
  • Maintain electronic medical records
  • Work under the direction of a Case Manager RN
