Job description

San Diego's St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul's PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.
This is a Full-time position with a typical schedule of Monday - Friday, 8:00AM - 4:30PM.
Job Summary: The Triage Registered Nurse (RN) is responsible for providing nursing services to assigned patients. RNs provide direct patient care, collaborate with other Interdisciplinary Team (IDT) members within the care team, and with other health care providers across various care settings.
Patient care provided by RNs is delivered in a manner that promotes an interdisciplinary team approach, respects patients' and families' goals for care, and meets Medicare/Medicaid conditions of participation and accreditation requirements. The RN practices in alignment with the organization's mission, values, and clinical philosophy. Upon request, participates in all areas of clinical services.
Duties and Responsibilities

  • Comprehensively assesses and anticipates all aspects of the patient's clinical nursing needs, pain, and other symptoms and, in collaboration with the primary care physician, develops an individualized plan of care PRN.
  • Incorporates participant/ caregiver preferences and goals within the plan of care.
  • Supports and respects the participant and/or caregivers' unique diversity, including spiritual and cultural beliefs.
  • Completes documentation accurately and completely as per agency standards and in compliance with regulatory standards.
  • Provides patient and family education as appropriate; areas of education include, but are not limited to goals of care, personal care, medications, procedures, safety, diet and nutrition, disease process, death, and dying.
  • Contributes to the team, utilizes team resources, supports team decisions, and maintains positive working relationships with all team members.
  • Works collaboratively with volunteers and provides ongoing support and guidance.
  • Demonstrates effective verbal, written, and electronic communication skills; gives and receives feedback in a timely, open manner; engages collaboratively in conflict resolution processes as needed.
  • Is flexible and responsive in meeting and exceeding the expectations of all customers, both internal and external.
  • Participates in mandatory educational activities and required meetings.
  • Is competent in using agency information and technology systems.
  • Performs in a supportive role to the IDT - Case Managers, answering calls coming into the agency with response to the telephone call by delegating tasks.

  • Travel to multiple work locations

Qualifications and Requirements:
  • Education
    • Current RN license in the state of California
    • BSN or higher degree preferred
  • Experience
    • Minimum of one year RN experience - PACE (Program of All-Inclusive Care for the Elderly), clinic, hospice, or home health experience preferred
    • Previous experience as a telephone triage nurse preferred
