Hospice Support Service Manager Jobs (NOW HIRING)

This full-time, remote position based in Pleasantville, New Jersey, offers a competitive salary range of $75,000 to $80,000 annually. The role of LCSW is designed to be remote, allowing you the flexibility to manage your schedule while focusing on compassionate care delivery. • Conduct comprehensive evaluations and assessments for patients to determine their psychological and social needs.
• Collaborate on the development and implementation of Individualized Family Service Plans (IFSP) tailored to meet the unique needs of families.
• Actively monitor and report on patient outcomes, ensuring continuous improvement of service delivery.
• Provide ongoing support and education to families, empowering them with strategies to enhance their loved one’s quality of life.
• Utilize evidence-based practices while adhering to Natural Learning Environment Practices to facilitate effective interventions.
• Build strong, supportive relationships with children, families, and team members to create an environment conducive to healing and growth.
• Maintain compliance with all applicable Federal and State regulations, contributing to our organization's reputation for excellence.
• Engage actively in professional development opportunities to enhance your skill set and effectiveness in supporting families.

In this role, you will primarily focus on providing care and support to families dealing with the emotional and psychological challenges associated with hospice care. Your efforts will particularly center around supporting young children and their families during a critical phase of their lives, ensuring they receive the dignity and respect they deserve. Your sensitivity and understanding will be pivotal in helping these families navigate complex emotional waters.

• A Master’s Degree or Doctorate in Social Work from an accredited institution.
• Current licensure as a Clinical Social Worker (LCSW) issued by the Department of Public Health.
• A valid driver's license to facilitate travel when necessary.
• Demonstrated experience in adapting therapeutic approaches to cater to diverse learning styles.
• Strong organizational, time management, and communication skills.
• Respect for diverse perspectives and a commitment to collaborative teamwork.
• Ability to develop rapport with patients and families, fostering trust in challenging situations.

• At least one year of experience working with children and families in early intervention settings.
• Experience working within a hospice, palliative care, or similar supportive care environment.
