We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join Lifescapes Counseling, LLC, as a Hospice Support Specialist. In this full-time remote position based in Elk Grove, California, you will play a vital role in providing comprehensive mental health services within a hospice setting, making a profound difference for patients and their families during a challenging time.

Key Responsibilities
  • Conduct thorough assessments of clients to identify their psychological, emotional, and social needs, and develop personalized care plans that align with their goals for hospice care.
  • Utilize evidence-based interventions to manage challenging behaviors, ensuring that clients feel safe, understood, and supported throughout their hospice journey.
  • Facilitate individual and group therapy sessions, employing therapeutic modalities such as cognitive-behavioral therapy (CBT) or supportive therapy to enhance clients' coping skills and emotional well-being.
  • Provide educational resources and support to families, addressing their concerns and equipping them with tools to encourage their loved one's progress and comfort.
  • Maintain meticulous clinical documentation in accordance with regulatory standards and organizational policies, ensuring comprehensive longitudinal care records for all clients.
Patient Population / Client Focus

Our patient population primarily consists of individuals facing terminal illnesses and their families, who necessitate emotional and psychological support. As a Hospice Support Specialist, you will work collaboratively with patients from diverse backgrounds and varying experiences, focusing not just on their clinical needs, but also respecting their values and wishes during end-of-life care.

Qualifications
  • A Masterโ€™s degree in Social Work (MSW) or a related discipline from an accredited institution.
  • Current licensure as a Clinical Social Worker in the state of California.
  • A minimum of two years of clinical practice experience, with a strong background in hospice, palliative care, or similar environments.
  • Demonstrated proficiency in clinical documentation and familiarity with electronic health record (EHR) systems.
  • Exceptional organizational, time management, and communication skills, with the ability to work effectively in a fast-paced environment.
Preferred Experience

While the minimum requirement includes two years of experience, we highly value candidates who have experience in multidisciplinary team settings, a background in trauma-informed care, or experience working with grief and bereavement counselors. Familiarity with local outpatient therapy resources and community-based services is considered a plus.

Compensation and Benefits

We offer a competitive salary range of $110,000 to $130,000 per year, commensurate with experience and qualifications.
