You will work closely with patients, families, and interdisciplinary teams to create and implement comprehensive care plans tailored to meet the unique needs of each individual. Responsibilities
  • Assess patients' health care needs and develop individualized care plans.
  • Coordinate patient care with physicians, family members, and other healthcare professionals.
  • Ensure compliance with state and federal regulatory requirements in hospice care.
  • Monitor patients' symptoms, medication responses, and overall progress.
  • Provide education and emotional support to patients and their families.
  • Facilitate and attend interdisciplinary team meetings to discuss patient care.
  • Document patient care accurately and in a timely manner in accordance with company policies.
Requirements
  • Valid RN license in the state of practice.
