Key Responsibilities

As a Hospice RN Case Manager, you will:

  • Manage a patient caseload and lead the full nursing process: assessment, admission, care planning, implementation, and evaluation
  • Deliver holistic patient care, addressing physical, emotional, psychological, social, and spiritual needs
  • Collaborate with the interdisciplinary care team, including physicians, social workers, and chaplains
  • Coordinate care with attending physicians and external providers
  • Conduct routine patient visits and update individualized plans of care
  • Provide end-of-life care, including clinical pronouncement and family support
  • Maintain accurate, timely documentation in compliance with state, federal, and company standards
  • Supervise and oversee care delivery by Home Health Aides (HHAs)

Qualifications
  • Active Registered Nurse (RN) license in Ohio (or compact state if applicable)
  • Graduate of an accredited nursing program
  • Current CPR and First Aid certification
  • Knowledge of quality assurance and hospice care standards
  • Valid driverโ€™s license and auto insurance
