Hospice Registered Nurse

Looking for meaningful nursing work with flexibility? The Care Team Hospice is seeking a compassionate Full-Time Registered Nurse to provide exceptional end-of-life care to patients in facility settings and private homes. Join a supportive team environment where you can make a meaningful impact while delivering compassionate, patient-centered hospice care to patients and families during life’s most important moments.

Schedule, Hours & Territory
• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Territory includes West Branch, MI, and surrounding areas within approximately a one-hour radius based on patient census and branch needs.

Key Responsibilities
 • Conduct comprehensive patient assessments and develop individualized plans of care
 • Provide skilled nursing care in accordance with hospice standards and physician orders
 • Coordinate care with physicians, caregivers, and interdisciplinary team members
 • Educate patients and families on symptom management, disease progression, and end-of-life care
 • Document assessments, interventions, and care plans accurately and on time in the EMR HCHB (Homecare Homebase)
 • Participate in interdisciplinary team meetings and care planning
 • Support after-hours patient needs as part of an on-call rotation when applicable

 • Active Registered Nurse (RN) license
 • Hospice or home health experience preferred
 • Strong clinical assessment and care coordination skills
 • Ability to work independently in the field
 • Strong communication and documentation skills
 • Experience using HCHB (Homecare Homebase) preferred

Field Role Requirements
 • Active driver’s license
 • Ability to pass a criminal background check
 • Ability to pass a Motor Vehicle Record (MVR) check
 • Current TB testing in accordance with agency policy
 • Current CPR certification as required for the role
