Now Hiring - Intake Coordinator 

The Intake Coordinator is responsible for, but not limited to:

  1. Completes initial inquiry forms for all potential admissions, and forwards completed forms to the appropriate Hospice Nurse and Director of Clinical Services and/or Administrator
  2. Insures that whenever possible, all referrals are converted to admissions
  3. If referrals do not convert to admissions, the Administrator will be notified
  4. Track all inquiries and referrals daily. This report will be given to the Administrator weekly
  5. Assists patients, families and responsible parties in completing the admissions packet as needed
  6. Works collaboratively with appropriate staff to insure written or verbal consents are obtained
