RN On-Call Nurse Responsibilities
  • Provide hospice nursing care through assessment, planning, implementation, and evaluation of patient care.
  • Perform on-call visits and respond to patient needs during assigned weekend shifts.
  • Assess and manage pain and symptoms while providing compassionate end‑of‑life care.
  • Collaborate with physicians, the Medical Director, interdisciplinary team members, and other healthcare providers to coordinate patient care.
  • Complete initial and ongoing patient assessments to determine hospice eligibility and care needs.
  • Regularly re‑evaluate patients and update individualized care plans.
  • Initiate appropriate preventive and palliative nursing interventions.
  • Educate and support patients and families throughout the hospice journey.
  • Communicate changes in patient condition promptly to the physician and care team.
  • Maintain accurate and timely clinical documentation in accordance with company policies and Medicare regulations.
  • Administer medications in accordance with federal and state regulations.
  • Coordinate durable medical equipment and other patient care needs.
  • Complete comprehensive death documentation when appropriate.
  • Participate in weekend on‑call coverage as assigned.
