We are seeking a Mental Health Professional / Housing Coordinator who is motivated to help individuals and families navigate life's challenges and achieve long-term wellness. This position is located at the Department of Mental Health, Charleston Dorchester Mental Health Center, 2100 Charlie Hall Blvd, Charleston, SC 29414.
Under limited supervision, this position serves as the Center's Housing Coordinator to ensure patient access to appropriate community-based housing. Completing assessments of patients living in Community Care Residential Facilities (CRCF's). This position also assesses the patient's psychiatric status to determine the ability to move to a lower level of care in the Housing continuum.
Responsibilities for the Mental Health Professional:
  • Provide oversight for the Center's housing programs, including Community Housing, Home share, Home share Respite, TLC/Housing and Homeless Program Supportive Housing developments, Community Residential Care Facilities (CRCFs), and other supportive housing programs, as applicable.
  • Coordinate administrative requirements of the agency's housing programs, including payment contracts and patient agreements, outreach and recruitment efforts for Home share providers, and completion of assessment for patients living in CRCFs.
  • Assess patients psychiatric status to determine ability to move to a lower level of care in the Housing continuum.
  • Coordinate with the agency's Transition program staff, including Clinical Care Coordinators (CCCs) and Transition specialists, MCO CCCs, Certified Peer Support Specialists, and Benefits/Entitlement Specialists. Partner with the local HUD Continuum of Care, public housing authorities, and affordable housing owners and property managers. Coordinate patient housing referrals and move-ins and maintain wait lists for each Center housing program.
  • Enter and maintain Community Housing data in the agency's Electronic Health Record. Prepare and submit monthly Community Housing report.
  • Enter and maintain Center Home share program and patient information in the agency's SharePoint site and CIS/Electronic Health Record.
  • Participate in other OMH or partner agency programs and activities as may be required.

Minimum and Additional Requirements
  • A master's degree in Guidance, Counseling, or the Social Sciences
  • Professional experience in human services or social service programs.
  • Licensed or eligible for licensure in SC and willing to work towards licensure within the first six (6) months of employment.
  • Must be able to meet BHDD-OMH credentialing requirements.

Additional Requirements:
  • Ability to lift 5lbs. and drive a state vehicle.
  • This position is considered essential and may be called back during crisis/disaster/emergencies.
  • Able to work a flexible work schedule.
  • Services are delivered in the community and the patient's homes.

Preferred Qualifications
  • Two (2) years of post graduate experience in the clinical counseling program.
  • Bilingual in English and Spanish (or another language) is a plus.

Additional Comments
Post hire, employees must:
  • Have knowledge of psychopathology and client management with the community-based psychiatric setting.
  • Have experience working with chronically mentally ill adults and seriously emotionally disturbed children and youth.
  • Maintain organizational and communication skills required for this position.
  • Have considerable knowledge of methods and practice management principles.
  • Have the ability to interpret and apply rules and regulations.
  • Have the knowledge and ability to establish and maintain effective working relationships.
