Experienced Bilingual Home Care Coordinator
Samash Holistic Homecare – Connecticut
Full-Time | In-Person/Hybrid Depending on Experience
Position Summary
Samash Holistic Homecare is seeking an experienced, highly organized, and dependable Bilingual Home Care Coordinator (English/Spanish) to support the daily operations and continued growth of our homecare agency.
The ideal candidate will have prior experience working for a homecare or home health agency and understand caregiver scheduling, client intake, staffing, documentation, compliance, referral coordination, and communication with clients, families, caregivers, and healthcare professionals.
We are specifically looking for an individual who can work independently, solve problems quickly, and help ensure that clients receive reliable and consistent care.
Responsibilities
Coordinate caregiver assignments and maintain accurate client schedules.
Respond promptly to caregiver call-outs and arrange replacement coverage.
Assist with new client intake, onboarding, and coordination of services.
Communicate professionally with clients, families, caregivers, nurses, referral sources, and other healthcare professionals.
Recruit, screen, and assist with onboarding qualified caregivers as needed.
Maintain employee credentialing and compliance documentation.
Monitor caregiver attendance, punctuality, availability, and performance.
Assist with insurance verification, authorizations, and payer-related documentation when applicable.
Maintain accurate client and employee records.
Assist with referrals from hospitals, rehabilitation facilities, skilled nursing facilities, case managers, social workers, and community organizations.
Follow up with prospective clients and referral sources.
Help coordinate admissions and start-of-care arrangements.
Participate in the agency's on-call/after-hours coordination rotation when required.
Maintain confidentiality and comply with HIPAA and agency policies.
Assist management with administrative and operational duties necessary for the efficient operation and growth of the agency.
Required Qualifications
Bilingual English/Spanish – required
Minimum 2 years of homecare, home health, healthcare staffing, intake, or care coordination experience preferred
Previous experience with a Connecticut homecare/home health agency strongly preferred
Strong caregiver scheduling and staffing experience
Excellent telephone and customer-service skills
Strong organizational and time-management abilities
Ability to handle multiple priorities and urgent staffing situations
Comfortable communicating with clients, families, caregivers, nurses, and referral partners
Proficient with computers, email, Microsoft Office, and electronic scheduling/documentation systems
Knowledge of HIPAA and healthcare confidentiality requirements
Professional, dependable, resourceful, and able to work independently
Preferred Experience

$22.00–$27.00 per hour, depending on experience, qualifications, and demonstrated knowledge of homecare operations.
