The Care Team Hospice is seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join our growing team!! This is an opportunity to make a meaningful difference by providing hands-on personal care and comfort to hospice patients while supporting families through life's most important moments.
As a Hospice CNA, you'll work alongside an experienced interdisciplinary team to help patients maintain dignity, comfort, and quality of life in their homes and care facilities. Key Responsibilities
• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient's individualized plan of care.
• Observe, document, and promptly report changes in patient condition to the supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.
• Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase).
Qualifications
• Active Certified Nursing Assistant (CNA) certification
• Previous CNA experience required; hospice, home health, long-term care, or skilled nursing experience preferred.
Field Role Requirements
• Valid driver's license, reliable transportation, and current automobile insurance.
• Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
