Registered Nurse Case Manager

Location: Barre, Vermont

Pay Rate: $2,300 To $2,560/Week

Contract: 16 Weeks

Schedule: Day Shift, Thursday–Monday, 36 Guaranteed Hours/Week

Experience: Minimum 1 year of Home Health RN experience required

About the Role

We are seeking a compassionate Home Health Registered Nurse (RN) for a 16-week contract opportunity with Central Vermont Home Health & Hospice in Barre, Vermont. This role is ideal for a dedicated nurse who is passionate about delivering individualized, patient-centered care in the home setting while serving as a Case Manager to coordinate comprehensive care plans.

Responsibilities

  • Provide skilled nursing care and comprehensive patient assessments in the home setting.

  • Develop, implement, and evaluate individualized care plans based on physician orders.

  • Serve as the primary Case Manager and coordinate care with physicians and the interdisciplinary team.

  • Educate patients and caregivers on disease management, medications, and home care needs.

  • Document patient care accurately using Homecare Homebase (HCHB).

Requirements

  • Active Vermont RN License required.

  • Minimum 1 year of Home Health RN experience.

  • Current BLS/BCLS Certification required.
