Job description

We are now hiring an Assessment Team Member for our Homecare division. This is a PRN/part-time opportunity based in our corporate office. We offer a flexible schedule, no on-call, and a weekly pay!!


Homecare Assessment Member Responsibilities

  • Acts as a sales person interfacing with referral sources, outside agencies, health professionals, and field staff.
  • Communicates with the client and family regarding arrangements for the initiation of services, insurance coverage, private payment, and scheduling.
  • Assisting with on-call rotation after hours (answering phone calls 1-2 nights per week and putting notes in report)
  • Gives timely and detailed reports to the office of what care is to be delivered and the best caregiver to assist the client. 
  • Completes a non-medical care plan detailing ADL's and iADL's that will benefit the client's life, while suggesting HomeLife Enrichment Program tasks in order to make a positive impact in the client's life. 
  • Willing to use personal cell phone device and personal data to document and fill out forms. A stylus pen and training on programs will be provided.
  • Willing to gather financial information from patient/family and in agreement to keep it secure.
  • Comfortable in homes that may have multiple pets or smokers present.
  • Comfortable conducting in home safety evaluations. This may include asking very personal questions to the client and family. 
  • Must possess general knowledge of healthcare professions and services including RN, PSN, PT, OT, SLP, SW, HHA, CNA, NP, PA, etc. in order to coordinate or recommend care with these professionals.
  • Must possess general knowledge on safe transfers and common diagnoses affecting the senior citizen population.
  • Agrees to look professional and clean at every patient meeting, and refrain from wearing scented products.
  • Willing to drive in and around Kalamazoo County and possibly surrounding counties for assessments, flyer distribution, and marketing events. Must have reliable transportation. Mileage is reimbursed.


Homecare Assessment Member Essential Requirements

  • Strong Healthcare backgrround
  • LPN or RN license (prefered)
  • High school diploma
  • Some college or medical training (preferred)
