Monday–Friday, 8:30 AM–5:00 PM
Location: Newburgh, NY
Job Type: Travel contract / Local Contract
Contract Length: 13
Pay Range: $52 to $58/ hr on W2

*Pay offered is based on experience, expertise, credentialing, and education.
Duties:
  • Provide skilled nursing care to patients in their homes.
  • Perform comprehensive patient assessments and identify clinical needs.
  • Develop and implement individualized plans of care.
  • Monitor patient conditions and communicate changes to physicians and the care team.
  • Administer medications and provide treatments as ordered.
  • Educate patients and families regarding medications, disease management, safety, and self-care.
  • Complete accurate and timely clinical documentation.
  • Coordinate care with physicians, therapists, aides, and other healthcare professionals.
  • Participate in case conferences and care coordination activities.
  • Meet the 30-point weekly productivity expectation.
  • Participate in required weeknight and weekend on-call coverage.
  • Travel locally between patient visits
Requirements:
  • Education: ASN or BSN
  • License(s):Active RN license
  • Certification(s):BLS
  • Years of experience: 2
  • Skills/Specialties/Technology: Home Health experience, CHHA experience, Homecare Homebase (HCHB) EMR experience
