What You'll Do
As a Home Health Registered Nurse, you'll provide skilled nursing care that allows patients to recover safely and comfortably in their own homes. You'll partner with physicians, therapists, caregivers, and interdisciplinary team members to develop personalized care plans that improve outcomes and enhance quality of life.
Responsibilities include:
  • Complete comprehensive patient assessments and ongoing evaluations.
  • Develop, implement, and adjust individualized plans of care.
  • Educate patients and caregivers on disease management, medications, treatments, and self-care.
  • Monitor patient progress and identify changes in condition.
  • Collaborate with physicians and the interdisciplinary care team to coordinate services.
  • Accurately document clinical findings and care provided.
  • Advocate for patients while promoting independence and safety at home.
  • Maintain compliance with all federal, state, and organizational standards, including HIPAA.

What You'll Bring
  • Current, unrestricted Registered Nurse (RN) license
  • Minimum of one (1) year of nursing experience in a clinical setting
  • Excellent assessment, communication, and critical thinking skills
  • Compassionate, patient-centered approach to care
  • Ability to work independently while collaborating with an interdisciplinary team
  • Reliable transportation, valid driver's license, and current auto insurance
  • Willingness to travel throughout the assigned service area
  • Physical Requirements: Sit, stand, bend, lift, and move intermittently throughout the workday. Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.

Preferred Qualifications
  • Associate or Bachelor's Degree in Nursing
  • Previous Home Health or Hospice experience
