Key Responsibilities – RN / Registered Nurse / Home Health

  • Perform in-home assessments and develop individualized care plans
  • Provide skilled nursing care under physician-directed orders
  • Administer wound care, injections, medications, and other treatments
  • Accurately document patient status, care plans, and progress notes
  • Educate patients and families on procedures and health management
  • Collaborate with physicians and care teams to ensure continuity of care
  • Coordinate care and lead clinical efforts as an RN Case Manager
  • Conduct tasks such as specimen collection, pain management, and disease monitoring
  • Ensure accurate ICD-10 coding and documentation for compliance and billing


Qualifications – RN / Registered Nurse / RN Case Manager

  • Active RN license in good standing in Indiana
  • Minimum of 1 year RN experience (home health or clinical setting preferred)
  • Strong interest in growing into a Case Manager or RN Case Manager role
  • Skilled in wound care, geriatrics, pediatric care, and behavioral health
