About the Role:
Join TRUCARING INC as an RN Case Manager in our Home Health Agency, where you will play a vital role in providing compassionate care to patients in the comfort of their homes. Responsibilities:
  • Conduct comprehensive patient assessments to develop personalized care plans.
  • Coordinate and manage patient care activities across interdisciplinary teams.

  • Provide education and support to patients and their families regarding health management.
