Your day-to-day responsibilities will include a variety of crucial tasks aimed at improving patient outcomes:

• Conduct comprehensive physical evaluations to ascertain each resident’s unique capabilities and therapeutic needs.
• Create and implement individualized treatment plans that include exercises, manual therapy, and specialized modalities aimed at enhancing strength, mobility, and functional independence.
• Administer therapy sessions, utilizing evidence-based practices and the latest therapeutic techniques to address specific physical impairments.
• Diligently monitor and document patient progress, making necessary adaptations to treatment plans to optimize outcomes.
• Engage in regular collaboration with an interdisciplinary team including physicians, nurses, and occupational therapists to ensure holistic patient care.
• Educate patients and their families on home exercise regimens and health strategies to facilitate ongoing improvement beyond therapy sessions.
• Ensure adherence to state and federal regulations as well as professional standards, while maintaining high-quality care.

In this role, you will primarily focus on a diverse patient population in home care settings, including individuals recovering from surgeries, managing chronic conditions, or overcoming injuries. The ideal candidate must possess the following qualifications:

• A current Physical Therapist license in New York State.
