AGENCY OVERVIEW

Centers For Family Development, Inc. (CFDI) is a private, non-profit comprehensive family counseling agency that was originally established in 1978 as Black Family Development, Inc. (BFDI) by the Detroit Chapter of the National Association of Black Social Workers (DNABSW). By establishing CFDI as a family counseling agency, DABSW sought to promote and provide quality community-aligned social work services in Detroit. Since that time, CFDI has grown to accommodate the in­creasing demand for a variety of specialized family-focused counseling and services in the community.

CFDI’s programming includes early childhood resources and literacy development, a 3-tiered juvenile justice service array, mental health prevention and treatment, substance use disorder prevention and treatment, R.E.A.L Brothers supportive resources, school-based intervention, and trauma-informed care. Most importantly, our staff are heroes who strive to match passion for service with equal parts best practice to partner with families to achieve their goals.


GENERAL RESPONSIBILITIES

Responsible for maintaining a caseload of youth in the Juvenile Justice system and their families. Responsible for youth/family assessments, service planning, case management (locating, linking and monitoring youth/family in community-based treatment and rehabilitative services), and meeting all youth/family needs according to the court order and other assessed treatment needs. Responsible for court attendance and reports. The Case Manager must maintain required in person contact schedule with youth and families based on probation level requirements and the needs of the youth, and coordination of care with assigned treatment providers.


DUTIES AND RESPONSIBILITIES

· Provides, assessment, case planning and counseling for individuals and groups.

· Attends staff meetings, trainings, workshops, and conferences.

· Provides outreach service along with all other service modalities.

· Maintains case records and related data, reports, forms.

· Makes appropriate referrals and follow-ups.

· Provides individual, family and group counseling using the home-based model.

· Provides transportation services to clients as needed.

· Provides assistance for families with emergencies as needed, i.e. food, clothing shelter, etc.

· Submits all program billing and required documentation in an appropriate and timely manner.

· Performs client advocacy services, and links with support services.

· Assists in the development of programs and services.

· Performs other duties as assigned.
