As a Home Health Registered Nurse, here’s a big-picture view of what you’ll do:

  • Provide one-on-one, home-based care for an adult patient, including tracheostomy (trach) and gastrostomy tube (G-tube) management, and medication administration
  • Monitor vital signs, assess patient conditions, and document findings accurately in patient records
  • Maintain open communication with the RN supervisor, care team, and family members to ensure continuity of care
  • Carry out the patient’s plan of care, including therapeutic interventions and rehabilitation exercises as prescribed
  • Educate the patient and family on the care plan, medication administration, and home safety measures
  • Follow healthcare regulations, infection control protocols, and ensure proper maintenance of medical equipment

A few must-haves for Home Health Registered Nurses:

  • Graduate of an accredited nursing program and active RN license in CA
  • Minimum of 1 year of nursing experience, ideally in home healthcare, skilled nursing, or an acute care setting
  • CPR certification
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing specialty, strong communication skills and compassion
