Job Summary
We are seeking a compassionate and skilled Registered Nurse (RN) to join our team. This role involves planning, organizing, and coordinating home health services, with a strong emphasis on community health and patient education.
Key Responsibilities
  • Develop and initiate individualized care plans in collaboration with patients and caregivers
  • Deliver physician-ordered nursing services and implement preventive and rehabilitative procedures
  • Educate patients and caregivers to promote health and independence
  • Identify and implement discharge planning needs
  • Coordinate necessary equipment and services
  • Supervise LVNs and HHAs

Qualifications
  • Active California RN license
  • Minimum 1 year of direct patient care experience (home health preferred)
  • Current CPR certification
  • Valid driver's license and insured, reliable vehicle
