Job description

Clinical Care Registered Nurse - Nursing

Schedule: Days M-F from 8:00 - 4:30

Assignment Details:

  • Contract Weeks: 13
  • Guaranteed Hours: 40

Requirements:

  • Bachelor's degree preferred in Science of Nursing; Associate's degree required.
  • RN Nursing Diploma or Degree from an accredited School of Nursing or University.
  • Minimum of one year of experience working with the frail, elderly, or long-term care population.
  • Minimum of one year home care experience preferred.
  • Minimum of one year wound care experience preferred.
  • Current CPR certification or willing to obtain.

Roles & Responsibilities:

  • Provide direct nursing care to participants, utilizing advanced nursing knowledge and skills.
  • Triaging emergent situations and managing complex clinical situations independently.
  • Monitor acute and chronic medical conditions in coordination with Primary Care Providers (PCPs).
  • Utilize care plans independently for clinic and home-based direct care.
  • Administer treatments, procedures, and medications as prescribed.
  • Conduct regular home visits to provide care and treatment as ordered by the PCP, including wound care, lab draws, and nurse assessments.

Compliance / Notes:

  • Must have reliable transportation, a current valid driver's license, and car insurance.
  • Must be 18 years of age or older.
  • Must be fully vaccinated, or willing to be fully vaccinated against specified communicable diseases.
