Job description

The Registered Nurse (RN) in Home Health Care delivers essential nursing services to patients in their residences, focusing on individualized care that supports patient recovery and well-being. This role requires proficient clinical skills, compassionate patient interaction, and effective coordination with healthcare teams to ensure high-quality home-based medical care.

Responsibilities

  • Perform comprehensive patient assessments to evaluate health status.
  • Develop and implement personalized care plans based on patient needs.
  • Administer medications accurately and monitor patient responses.
  • Provide expert wound care and monitor healing progress.
  • Educate patients and families on health management and treatment protocols.
  • Monitor vital signs and recognize changes requiring intervention.
  • Conduct home visits to deliver nursing care in the patient’s environment.
  • Coordinate care with physicians, therapists, and other health professionals.
  • Maintain thorough and accurate documentation of patient care and progress.
  • Respond promptly to emergencies and provide critical care as needed.
  • Implement infection control procedures to ensure patient and caregiver safety.
  • Communicate effectively with patients, families, and healthcare teams.

Required Qualifications
