The Registered Nurse (RN) in Home Health Care delivers essential nursing services to patients in their residences, focusing on individualized care that supports patient recovery and well-being. This role requires proficient clinical skills, compassionate patient interaction, and effective coordination with healthcare teams to ensure high-quality home-based medical care.
Responsibilities
- Perform comprehensive patient assessments to evaluate health status.
- Develop and implement personalized care plans based on patient needs.
- Administer medications accurately and monitor patient responses.
- Provide expert wound care and monitor healing progress.
- Educate patients and families on health management and treatment protocols.
- Monitor vital signs and recognize changes requiring intervention.
- Conduct home visits to deliver nursing care in the patient’s environment.
- Coordinate care with physicians, therapists, and other health professionals.
- Maintain thorough and accurate documentation of patient care and progress.
- Respond promptly to emergencies and provide critical care as needed.
- Implement infection control procedures to ensure patient and caregiver safety.
- Communicate effectively with patients, families, and healthcare teams.
Required Qualifications
- Valid Registered Nurse (RN) License.
Preferred Qualifications
