This full-time position allows for a flexible remote work environment, enabling you to provide care to patients from the comfort of their homes while maintaining a collaborative relationship with our hospice team. You will engage with colleagues through virtual tools and participate in scheduled meetings to discuss patient progress and care strategies. • Conduct thorough psychosocial assessments aimed at evaluating the mental health status, strengths, and specific needs of patients, focusing on their emotional and psychological well-being.
• Utilize evidence-based therapeutic techniques, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), to support individuals and families coping with grief and anticipatory loss.
• Collaborate closely with interdisciplinary teams, including psychiatrists, nurses, and palliative care specialists, to ensure that all aspects of patient care are integrated and cohesive.
• Educate patients and their families regarding diagnoses, care strategies, treatment options, and medication management, empowering them with information and support.
• Facilitate individual and group therapy sessions, as well as psychoeducational sessions designed to provide coping mechanisms tailored to the unique experiences of hospice patients.
• Develop and implement personalized treatment plans in consultation with the treatment team and align them with patient-centered goals and preferences.
• Help patients navigate the complexities of the healthcare system, providing advocacy for their rights, needs, and desires at every stage of the care process.
• Lead therapeutic group sessions, psychoeducation programs, and discharge planning meetings to foster a supportive and constructive environment for all participants.

This role primarily focuses on patients within the hospice care realm, who are often confronting life-limiting illnesses. As a Clinical Social Worker, you will be pivotal in delivering care that respects their dignity and emotional needs, as well as addressing the mental health challenges faced by their families during this sensitive time.

• A Master's degree in Social Work from an accredited program is required.
• Current, active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Comprehensive knowledge of mental health disorders, treatment modalities, and crisis intervention strategies is essential.
• Demonstrated ability to work effectively in a collaborative, multidisciplinary team setting.

• Experience working in a hospice or palliative care setting is highly desirable.
