Heart & Listen Clinical Counseling is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) for a full-time remote position, focused on delivering exceptional psychosocial support to clients in our hospice center located in Alpharetta, Georgia. • Conduct thorough psychosocial assessments through patient interviews, observations, and medical record reviews to gather detailed insights into emotional and social factors influencing patients’ care during their hospice experience.
• Deliver empathetic counseling and support services to both patients and their families, addressing adjustment issues related to terminal diagnoses and providing strategies for coping with grief and loss.
• Collaborate with our rehabilitation team to design and implement individualized treatment plans that incorporate psychosocial assessments, healthcare needs, and family dynamics.
• Assist patients in accessing community resources such as support groups, vocational rehabilitation services, and housing assistance tailored to their end-of-life care.
• Facilitate discharge planning, ensuring effective transitions for patients moving to home care or other supportive environments while also coordinating follow-up care and community resource education.
• Participate in regular team meetings to update care plans, share insights, and ensure cohesive communication within the interdisciplinary team focused on patient-centered care.

The patient population you will serve consists of individuals facing terminal illnesses and their families. Your role will directly influence their emotional and mental well-being, helping them navigate this significant life transition. You will provide individualized support to address not only the practical challenges but also the emotional complexities surrounding end-of-life issues, enhancing both patient and family engagement in care plans.

• A Master’s degree in Social Work from an accredited program is required.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
• Demonstrated expertise in addressing psychosocial issues, particularly within hospice or palliative care, is preferred.
• Strong communication and interpersonal skills are essential, enabling effective listening and guidance during sensitive discussions.
• Ability to maintain professionalism and compassion in challenging situations, along with a commitment to continuous professional development.

• Experience working in rehabilitation, hospice, or medical settings with a focus on end-of-life care is highly desirable.
• Familiarity with community resources available to patients and families facing terminal illnesses.
• Knowledge of grief counseling practices and adjustment strategies for patients and their support systems.

We offer a competitive salary range of USD 75,000 to 100,000 annually, adjusted based on experience and qualifications. In addition, successful candidates will receive a sign-on bonus as a welcome to our team.
