PRN Physical Therapist Opportunity

Seeking PRN Physical Therapists for both our Medicare and Hospital at Home patients servicing across Vero, Fort Pierce, Port St. Lucie, Stuart, Indian Town, Hobe Sound, Jupiter and surrounding areas. Job Responsibilities • Provide skilled physical therapy services to patients receiving hospital-level care at home and for Medicare Skilled Patients. • Perform comprehensive PT evaluations addressing: Strength, balance, endurance, and functional mobility Gait deviations and fall risk Transfers, bed mobility, and stair negotiation Activity tolerance and cardiopulmonary response • Develop and implement individualized, evidence-based treatment plans aligned with acute recovery and discharge readiness Address functional decline related to: Recent hospitalization Acute illness or injury Deconditioning and mobility loss Educate patients and caregivers on: Safe mobility techniques and transfer strategies Fall prevention and home safety Use of assistive devices and mobility equipment Monitor patient progress closely, adjust plans of care, and communicate changes promptly with the care team Collaborate with physicians, nursing, OT, and care management to support safe transitions and reduce hospital readmissions Complete timely, accurate documentation in compliance with Medicare, hospital-at-home, and agency standards Qualifications Required Active Physical Therapist (PT) license in the state of Florida Degree from an accredited Physical Therapy program (DPT preferred) Current CPR certification Valid driver's license and reliable transportation Strong clinical reasoning, communication, and time-management skills Preferred / Highly Desired Experience in one or more of the following: Acute care or hospital-based physical therapy Hospital at Home or transitional care programs Medicare Skilled home health physical therapy Skilled in: Gait training and balance rehabilitation Fall risk assessment and prevention Functional mobility and discharge planning Post-acute and medically complex patient care Comfortable practicing independently in a home-based acute care setting Familiarity with EMR documentation systems About Activa Home Health Activa Home Health delivers high-end, patient-centered care through innovative programs designed to meet patients where they are—including hospital-level care at home.
