Job description

SALARY: $72 - 75K DOE

OVERVIEW:

The Manager, Coordinated Care & Outreach will provide leadership to support the provision of disaster and crisis response services to individuals and families in Harris, Fort Bend, Montgomery, and Waller Counties.  The Manager will oversee and execute emergency and crisis care coordination for individuals by working with community social service providers to provide expedited assistance and ongoing resources to support their sustainability and resiliency.  The Manager develops and coaches direct reports, providing clear leadership through crisis management for individuals and families. 

PRINCIPAL DUTIES & RESPONSIBILITIES: 

  • Manage and conduct client care coordination services, including but not limited to outreach, home and community visits, eligibility screening, and intake for each client. Using a strengths-based approach, oversee the development of Individual Stabilization Plans to provide clients with intensive intervention, navigation and on-gong support and assistance in applying for and obtaining community resources to meet emergent or unmet needs.   Oversee follow-up calls and/or visits to monitor and assess the effectiveness of services provided. Ensure documentation of comprehensive client background information, case history, updates, and progress towards Individual Stabilization Plan in both physical and digital files via online software, ensuring maintenance of client confidentiality and personal identifying information.
  • Pilot and implement integrated models with area providers to provide holistic programming and client intake services for public benefits, housing, utility assistance, United Way’s integrated client journey program, governmental programs and others.  Act as lead for care coordination across multiple service providers, providing support to care coordinators and community navigators.  Regularly facilitate and convene meetings with agencies, recommend appropriate courses of action, establish planning guidelines, and ensure compliance with federal, state and county emergency management guidance during times of disaster.  Plan and conduct training and exercises to validate/enhance operations, plans and procedures. Integrate lessons learned into plans and procedures.
  • Provide on-going program evaluation utilizing program data, analyze program data, and provide recommendations to the AVP for continuous growth and quality improvement. Provide reports and status for all cases in their portfolio as needed by program demands.
  • Perform special assignments and other work, on an as-needed basis. A willingness to take on additional duties to ensure the success of the team is essential. Evening and weekend work with some local travel will be required. Participate in workshops, seminars, education programs and other activities that promote professional growth and development.
