Job description

 Job Summary

Home Care Now is seeking a compassionate and clinically strong Home Health RN/LPN to provide one-on-one care in a home setting. This role is ideal for a nurse who values autonomy, patient relationships, and the ability to deliver skilled care in a home-based setting rather than a facility environment.

What You’ll Do

  • Travel to patient homes to provide skilled nursing care
  • Assess patient condition, vital signs, and response to treatment
  • Administer medications 
  • Complete accurate, timely documentation for each visit

What We’re Looking For

  • Active RN license in the state of employment
  • Current CPR or BLS certification
  • Level 2 Background 
  • Strong assessment and communication skills
  • Ability to work independently in a home-based care setting
  • At least 1year of  Home health experience
  • Experienced with trach g-tube  

Schedule and Pay

  • Both days and Nights available 7am to 7pm/7pm to 7am
  • Flexible scheduling options may be available
  • Pay range: $32 to $42per hour
