We are seeking an experienced Home Health Intake Manager to lead a small, high-performing intake team supporting multiple home health agencies throughout California, including Choice Home Health, Focus Home Health, and San Diego Home Health. This role offers an exciting opportunity to help shape and optimize intake operations across a growing organization while directly impacting patient access, referral conversion, and agency growth.

This is a hands-on leadership role responsible for overseeing intake operations, referral management, insurance verification, admission readiness, and team performance while partnering closely with Clinical Operations, Business Development, Scheduling, and Agency Leadership.

What You'll Do

  • Lead, coach, and develop a team of Intake Care Coordination and Insurance Verification professionals.
  • Oversee referral intake, insurance verification, authorizations, and admission readiness.
  • Drive referral conversion, admission timeliness, and operational efficiency.
  • Partner with branch leaders, clinicians, and business development teams to support agency growth.
  • Monitor intake performance metrics and implement process improvements.
  • Ensure compliance with Medicare, payer, state, and agency requirements.
  • Serve as a key liaison between referral sources, branch leadership, and centralized intake operations.
  • Support standardization of intake processes and best practices across multiple agency locations.

What We're Looking For

Required

  • Minimum 3 years of Medicare-certified home health experience.
  • Minimum 2 years of leadership experience in intake, admissions, care coordination, referral management, or related healthcare operations.
  • Strong knowledge of Medicare eligibility, payer requirements, authorization processes, and home health admissions.
  • Excellent communication, organizational, and problem-solving skills.

Preferred

  • Bachelor's degree in Healthcare Administration, Nursing, Business Administration, or related field.
  • Experience with HomeCare HomeBase, Forcura, and other home health referral management platforms.
  • Experience supporting multi-site home health operations.

Travel Requirements

This position may be based anywhere within California. Regular travel is required to support agency locations, team members, and operational initiatives throughout the organization's service areas.
