Job description

Overview
The Home Health Department provides Personal Care Services to patients in Wake County and some of the surrounding counties. Our Intake Nurse is responsible for evaluating patients and overseeing the plan of care for PCS patients. The plan is based upon the patient's personal and health care needs identified through the assessment of the patient's physical, psychological, and socioeconomic status. The Intake Nurse understands the needs of the organization and supports the mission, values, and management of patient care services.
