Job description

Job Title

Home-Based Primary Care (HBPC) Physician

Duties

The Home-Based Primary Care (HBPC) physician practices in the outpatient setting in the Central California Health Care System and in the residence of the client. The physician is academically and clinically prepared to perform a broad spectrum of patient care services at the expert level. In addition to providing primary health care, the physician serves as a consultant to nursing and medical staff for patients with complex treatment or care needs in the home. The physician works collaboratively with other interdisciplinary team members in providing quality care and patient education, participating in research activities, and assisting patients in acute and chronic rehabilitation techniques.

  • Assessment, diagnosis, treatment and outcome measures of healthcare issues.
  • Provides medical care to patients with acute and chronic health problems.
  • Provides preventive medical care including screening and vaccinations.
  • Follow up with additional home visits as appropriate.
  • Collaborates with nursing personnel in the care of patients.
  • Other duties as assigned.
