Summary

We're looking for a Home-Based Support (HBS) Self-Direction Assistant (SDA)/QIDP to play a key role in assisting individuals and families in utilizing Home-Based Services funding to provide necessary support to Participants. This position requires a compassionate professional who can effectively coordinate services, manage documentation, and empower individuals with intellectual and developmental disabilities to thrive in their homes and communities.

The Role

  • Provide assistance with monthly and annual budgeting of allotted Home-Based Services funds.
  • Prepare and update annual budgets and service agreements to reflect participant needs and program changes.
  • Train and assist participants and families with timekeeping and other employer-related tasks required to interface with the Fiscal Management Agency.
  • Complete monthly billing case notes for Self-Direction Assistant services rendered.
  • Maintain required monthly, quarterly, and annual documentation to ensure compliance with Illinois Home-Based Services requirements.
  • Research and share information about beneficial services and resources with participants and their families.
  • Ensure all service providers meet the requirements for participation in Home-Based Services.
  • Travel to client homes to conduct meetings and provide in-person support as needed.
  • Attend meetings, in-services, and seminars as required.
  • Demonstrate competency with Lambs Farm's policies and procedures.

Qualifications

  • Bachelor's degree in Special Education or related human service field.
  • Minimum one year of experience working with individuals with intellectual/developmental disabilities.
  • Certification as a Qualified Intellectual Disabilities Professional (QIDP) or ability to complete QIDP training within 6 months from date of hire.
  • Valid driver's license and reliable transportation for travel to client homes.
  • Excellent written and verbal communication skills.
  • Proficiency in writing, speaking, and understanding English.
  • Proficiency in basic computer skills, including accurate typing, navigating websites, entering and recalling data, using word processors and spreadsheet software, managing emails, and familiarity with digital communication etiquette and techniques.
Compensation

The salary for this role is $48,880 annually.
