Build Independence Where It Matters Most — At Home

@Home Therapy is seeking a licensed Occupational Therapist to provide in-home services to adult and geriatric patients in the Great Neck area. This per diem opportunity is ideal for clinicians who value autonomy, flexible scheduling, and meaningful one-on-one care in a home setting.
In this role, you’ll focus on helping individuals safely navigate daily routines, regain functional abilities, and remain independent in their own environment.
What You’ll Be Doing:
Functional Therapy in the Home:
  • Assess patients’ ability to perform everyday tasks such as dressing, bathing, and meal preparation.
  • Identify environmental barriers and recommend modifications to improve safety and accessibility.
Individualized Treatment Approach:
  • Create targeted therapy plans that support independence and long-term function.
  • Incorporate adaptive techniques, equipment training, and energy conservation strategies.
Ongoing Patient Support:
  • Monitor progress and adjust interventions based on patient needs and goals.
  • Provide hands-on guidance to caregivers to reinforce therapy outcomes.
Documentation & Coordination:
  • Complete visit notes using a web-based EMR system.
  • Collaborate with interdisciplinary team members when needed.
