Job description

Description:

Care Alliance Health Center is seeking a School-Based Community Health Worker to serve as a trusted connection between students, families, schools, and Care Alliance Health Center.

This role supports student health and well-being by helping families access healthcare services, reduce barriers to care, and connect with medical, dental, behavioral health, vision, and community-based resources.

About the Role

The School-Based Community Health Worker works within assigned schools to provide outreach, care coordination, health education, resource navigation, and family support. This position partners closely with school administrators, nurses, counselors, teachers, community organizations, and Care Alliance staff to identify health-related and social barriers that may impact student attendance, engagement, and academic success.

This is a great opportunity for someone who is compassionate, organized, community-focused, and passionate about supporting students and families.

What You'll Do

Key responsibilities include:

  • Serve as Care Alliance Health Center’s primary Community Health Worker within assigned schools.
  • Build positive, trusting relationships with students, families, school staff, and community partners.
  • Promote awareness of Care Alliance’s medical, dental, behavioral health, vision, and supportive services.
  • Participate in school events, parent engagement activities, health fairs, wellness initiatives, and community outreach events.
  • Assist eligible students and families with enrollment into Care Alliance services.
  • Help students and families access medical, dental, behavioral health, vision, and specialty care services.
  • Coordinate referrals, appointments, and follow-up care to support continuity of care.
  • Conduct Social Determinants of Health screenings and assess barriers affecting student health and well-being.
  • Connect students and families with resources for transportation, food insecurity, housing instability, insurance coverage, financial assistance, and other social needs.
  • Support attendance initiatives by addressing health-related and social barriers that may contribute to chronic absenteeism.
  • Deliver age-appropriate health education to students and families.
  • Promote preventive care, oral health, behavioral health awareness, nutrition, physical activity, and healthy lifestyle behaviors.
  • Serve as a liaison between Care Alliance, schools, families, and community organizations.
  • Document outreach, care coordination, screenings, health education, and case management activities in the electronic health record and other required systems.

Why Join Care Alliance?

Care Alliance Health Center is committed to providing exceptional, accessible, and compassionate healthcare for all. As a School-Based Community Health Worker, you will help students and families overcome barriers, access needed services, and build healthier school communities.

We offer a competitive benefits package, including:

  • Paid Time Off
  • 10 paid holidays
  • Medical, dental, and vision insurance
  • Life insurance
  • Retirement plan with up to a 7% employer match

Work Environment

This position primarily operates within assigned schools and school-based health settings, with regular collaboration with Care Alliance staff and community partners. The role requires frequent interaction with students, families, school personnel, healthcare providers, and community organizations. Occasional evening or weekend participation in school or community events may be required.

Regular travel between assigned schools, Care Alliance Health Center locations, students’ homes when appropriate, and community partner organizations is required.
