Schedule: Monday-Friday 8:00am to 5:00 pm with occasional weekend and weekday afterhours call

Position Summary

The Home-Based Primary Care Physician provides direct patient care in patients' homes, assisted living facilities, and personal care facilities. This physician also serves as a clinical supervisor and subject matter expert for home-based primary care nurse practitioners practicing in the same settings. Responsibilities include evaluating and treating chronic conditions and urgent episodic illnesses and injuries; ordering and interpreting selected laboratory and diagnostic tests; making referrals to specialists as needed; prescribing medications; and performing procedures within the physician's scope of practice, established protocols, and available equipment.

Position Qualifications
  • M.D. or D.O. required.
  • Current licensure, or eligibility for licensure, to practice medicine in Kentucky, including DEA registration and controlled substance authority, required.
  • Fellowship training in Geriatrics is preferred but not required.
  • Board certified or board eligible, with board certification expected within five years of employment, required.
  • Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.
