Position Overview

We are seeking a compassionate and skilled professional to provide home-based therapeutic services to individuals and families in their communities. In this role, you will work collaboratively with clients, caregivers, and interdisciplinary partners to support meaningful progress toward treatment goals, strengthen coping skills, and promote overall well-being.

Responsibilities
  • Provide individualized therapy services in home and community settings.
  • Complete assessments, develop treatment plans, and monitor client progress.
  • Maintain regular communication with clients, families, and care teams.
  • Document sessions, treatment updates, and service notes accurately and on time.
  • Support clients in developing coping strategies, problem-solving skills, and healthy routines.
  • Collaborate with other providers and community resources to coordinate care.
  • Adhere to ethical standards, confidentiality requirements, and organizational policies.
  • Participate in case reviews, supervision, and professional development activities as needed.
Qualifications
  • Master’s degree in counseling, social work, psychology, marriage and family therapy, or a related field.
  • Active licensure or eligibility for licensure, depending on role requirements and state regulations.
  • Experience providing therapeutic services to individuals, children, families, or other client populations.
  • Strong clinical judgment, communication skills, and ability to build rapport with diverse populations.
  • Ability to work independently while managing caseload responsibilities.
  • Proficiency in documentation and familiarity with electronic health record systems.
  • Valid driver’s license, reliable transportation, and willingness to travel to client homes as needed.
Preferred Qualifications
  • Experience delivering in-home, community-based, or intensive family services.
  • Knowledge of trauma-informed care, crisis intervention, and evidence-based treatment approaches.
  • Experience working with multidisciplinary teams and community support systems.
  • Bilingual communication skills are a plus.
