Medical Records / Health Information Management (HIM) Coordinator

Full-Time | Monday–Friday | 8:00 AM–5:00 PM

Crowne Health Care is currently accepting applications for a Medical Records / Health Information Management (HIM) Coordinator within our long-term care and rehabilitation facility specializing in geriatric care.

This position is ideal for a dependable, detail-oriented professional who understands the importance of accurate documentation, regulatory compliance, confidentiality, and organized health information management. The HIM Coordinator plays an important role in maintaining complete and accurate resident medical records while supporting clinical, administrative, billing, and quality assurance processes.

Qualifications & Requirements

  • ICD-10 Certification Required
  • Experience in Medical Records or Health Information Management preferred
  • RHIT or RHIA credential preferred, or relevant HIM education/experience

Essential Job Functions

  • Organize, maintain, and monitor resident medical records to ensure accuracy, completeness, and regulatory compliance
  • Perform and assist with ICD-10 coding and diagnosis history maintenance
  • Ensure appropriate documentation is available to support coding and billing processes
  • Create, scan, index, and maintain documentation within the electronic medical record
  • Process medical records associated with new admissions, transfers, and discharges
  • Protect resident health information in accordance with HIPAA privacy and security requirements
  • Conduct routine medical record audits and documentation quality checks
  • Identify incomplete or inaccurate documentation and assist with appropriate follow-up and corrective action
  • Assist with forms management, document workflows, and medical record indexing
  • Support facility survey preparation and respond to medical records requests during surveys as needed
  • Participate in Quality Assurance and Performance Improvement (QAPI) activities related to documentation and health information
  • Educate and assist staff regarding documentation requirements, medical records procedures, and HIM processes
  • Maintain effective communication with nursing, administration, billing, and other interdisciplinary departments
  • Complete required reports and assessments related to the medical records/HIM function
  • Perform additional Health Information Management duties as assigned
